Support coordination will assist you to build the skills you need to understand, implement and use your NDIS plan. A support coordinator works with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community….

When your NDIS plan is approved, you might receive funded support coordination to help you to use your plan to achieve your goals.

Getting this type of funded support in your plan depends on your individual needs and circumstances, such as your disability or personal situation.

Your FOCAS support coordinator can help you in many ways, depending on your preferences and choices. You will have commitment and determination from your SC to be close in contact to assist with any questions and concerns and guide you to:

  • understand and use your NDIS plan
  • make the most of your funded supports and understand your budgets
  • access community and mainstream services
  • build your ability to become more independent
  • connect with people in your life such as family, friends, support workers and the community to help you achieve your goals
  • connect with providers of your choice
  • decide when you want to access supports and services, and how much you want to pay for these.
  • make decisions by providing you with options you can understand to ensure you receive safe and quality supports
  • organise reports/quotes and support you to prepare for your next plan review meeting and support/advocate with you during your review meeting

Build confidence and skills to manage your supports independently so you may not need support coordination in your next plan

They will also help you with your service agreements and service bookings and what they mean.

They will also closely liaise with your Plan Manager if Plan Managed or the NDIS if Agency managed to regularly check funds used and funds remaining in each of your funded support categories within your plan.

Please remember, if you are unhappy with any services/support you receive, you are in control of that choice.

This means you have the choice over WHO provides your supports WHEN you have chosen to receive supports and HOW the supports you have chosen are provided.

Your FOCAS support coordinator will always make sure you have choice and control over your goals and the services that provide your supports and will never make those decisions on your behalf
